The Ulster County Department of Health is considering offering a vaccination clinic on the Ellenville Central School District campus for children ages 12 and up. At the current time, the Pfizer vaccine is approved for children ages 16+ and it is expected
that children ages 12 - 15 will soon become eligible.
The purpose of this survey is to gather information from our families to determine interest. Parents are asked to please complete the survey to gather information and determine the level of interest.
