Families who are interested in providing home instruction to their children must submit a letter of intent to the Office of the Deputy Superintendent. This letter can be emailed to Justine Avery, submitted via mail, or dropped off at:

Upon receipt of your letter of intent, the information on this page will be sent as a Home Instruction Packet via mail.  

At the time of the filing of the fourth quarterly report as specified in your child’s Individualized Home Instruction Plan, the parent should also file an annual assessment. Please note that this is a summary. Refer to Section 100.10 of the Regulations of the Commissioner of Education for further details.

You may use the District-provided template or create your own. If you use your own template, it must include:

• Family contact information

• The child’s name, date of birth, age, grade level

• The plan of instruction to be used in each of the required subject areas. This must include the topics covered for the year for each required subject area

• The submission dates for the quarterly reports (more information below)

• The names of the individuals/program providing instruction

Note: You may use the District-provided template or create your own. If you use your own template, it must include:

• The child’s grade and quarterly report number.

• The number of hours of instruction during the said quarter.

• A description/list of topics covered in each subject area.

• A grade (letter or numerical) or written narrative evaluating the child’s progress,

• A written explanation in the event that less than 80 percent of the amount of the course materials as set forth in the IHIP planned for that quarter has been covered in any subject.
